D.A.P. expands case management services

(PALM SPRINGS, CA – October 27, 2009)

In an effort to better reach its 11,000 square mile service area, Desert AIDS Project has hired Cliff Snyder as a new bilingual case manager. Working out of County Public Health Department offices, Snyder will offer case management services two days each month in Hesperia and another two days each month in Perris to serve the needs of clients in the surrounding areas. The remainder of Snyder’s time will be spent at D.A.P.’s main campus in Palm Springs.

“Many of our clients come to us in Palm Springs for our HIV medical and dental clinics, as well as the variety of social services we offer out of our main office,” said David Brinkman, D.A.P. Executive Director. “But we are committed to serving our entire client base, wherever they may live.”

D.A.P. case managers first assess their clients’ mental, social, financial, and functional status, to determine their individual spectrum of needs. Then, on an ongoing basis, they will recommend and coordinate services such as financial counseling, public assistance, benefits counseling, housing, transportation, and much more. By coordinating the delivery of these services for clients, case managers take an active role in helping to maintain their clients’ health.
