Title: Program Development and Quality Improvement Manager
Reports To: Executive Director
Beginning Salary: $ 50,000 - $ 60,000 / yr.
Status: Full-time; Exempt
Position Purpose:

Under general direction, assists Department Directors in planning and organizing Desert AIDS Project patient care programs through the management of 1) quality management and quality improvement programs, 2) grant funding and 3) public policy.

  1. Plans, develops, implements and coordinates quality assessment/assurance and continuous quality improvement program. Responsible to develop, monitor and evaluate patient care programs which enable D.A.P. to assess and improve operations and patient health outcomes. Identifies and assesses problem areas related to patient care to include development and implementation of systematic solutions to such problems and subsequent monitoring, documentation and evaluation of these solutions. The position ensures that all programs are in compliance with applicable standards of external regulatory agencies and funding sources.
  2. Ensures fiscal health of programming by identifying and pursuing grant funding strategies.
  3. Manages policy and advocacy action network to focus on state, federal and local HIV public policy issues and represents D.A.P. in state, federal, local and other advocacy efforts
Position Responsibilities
Quality Management and Quality Improvement:
  • Working with Department Directors, sets program quality and performance standards;
  • Develops, directs and evaluates program outcome measures;
  • Oversees the coordination and implementation of organization wide task forces engaged in process analysis and performance improvement – continuous quality improvement teams; cultural competency improvement programs;
  • Plans and recommends policy and system initiatives incorporating clinical and services indicators;

Education:

Title: Health Education / Risk Reduction Counselor
Reports To: Community Support Counselor

Job Description:

Health Educator/Risk Reduction Counselor (H.E.R.R.) provides direct service provision to African Americans, Latinos and women who are at highest risk for HIV infection and/or currently living with HIV/AIDS within our service area. This includes development of client self-management improvement plans, informational counseling, advocacy, outreach and skill building workshops. H.E.R.R. works in D.A.P.'s Palm Springs and Indio offices and works closely with inter-agency departments such as the heath center, case management, and education. (Scheduling and outreach may require working evenings and weekends.

Counseling Responsibilities

Responsible for design and delivery of self management and skill building workshops (including topics on goal setting, problem solving, health literacy, nutrition, exercise, medication adherence, etc). Responsible for the development of a comprehensive inter-agency protocol which includes follow-up on linkages made, assessment of appropriateness of linkages and assurance that all clients needs have been, or are being met. Records all linkages made and follow up steps in client file. Conducts health literacy assessment and quality of life survey on each client.

Community Responsibilities:

Outreach and provide education about available services to community and health and social service providers serving vulnerable populations. Outreach and implement health education/risk reduction services to target populations using D.A.P.'s Indio and Palm Springs Community Centers.

Administrative Responsibilities:

Conducts program evaluation (process & outcome). Has direct involvement with data collection and entry and assures the confidentiality of all client data. All referrals should be followed-up with the client when reviewing action steps. Referral follow-up and services received must be documented in progress notes. Complete accurate documentation of client encounter. Performs program related functions such as maintaining participation logs, demographic data, completing required documentation, and participates in quality assessment activities.

Prerequisites
Must be bilingual Spanish
Must have a Bachelor's Degree in a related field or equivalent experience in HIV services. Experience in addiction treatment services preferred.
Must be able to clear a background check
Demonstrated ability and/or commitment to working effectively with people from diverse backgrounds, with sensitivity to ethnic, cultural, gender, and sexual minorities.

Title: Community Support Counselor
Reports To: Director of Social Services
Beginning Salary: 20.00 - $25.00 / hr.
Status: Part-time; Non-exempt
Summary:

The Desert AIDS Project Community Center is the gathering place for persons with HIV and their allies in Palm Springs and the surrounding areas. The Community Support Counselor provides psychosocial support services and coordinates all functions of the community space, including Early Intervention Services, implementation of the Self-Management Curriculum, information and referral services, the library, and the CyberCenter. The Community Support Counselor is responsible for planning, development, and implementation of programs that engage our local community in the work of Desert AIDS Project. The Community Support Counselor counsels, leads support groups, and encourages social interaction to combat social isolation.

Duties and Responsibilities:
Mental Health Services
Design and implement psychosocial support programs.
Provide group counseling sessions to DAP clients on various topics.

Community Center Management
Develop and implement annual plans to ensure that the Community Center is a welcoming, safe space for HIV community and general public.
Design & implement self-management programs based on the Chronic Care Model such as goal-setting, problem solving, health literacy, nutrition, exercise, cognitive techniques for symptom management, medication adherence and mitigation of side effects.
Develop individual self-management improvement plans
Review and revise programs offered to ensure community involvement.
Oversee specified aspects of the use of the community service wing for both in-house and community wide entities. 
Coordinate and facilitate programs, including but not limited to wellness, pastoral care, bereavement counseling, nutrition, fitness and complementary therapies available for people living with HIV.
Manage day-to-day operations of Community Center.
Recruit additional organizations to participate in the Community Center
Respond to community member suggestions, ideas, and complaints that relate to the Community Center.

Staff and Volunteer Supervision
Manage Health Education – Risk Reduction Counselor
In collaboration with Volunteer Services Associate, plan, implement and manage a comprehensive volunteer program to support community center programs.
Schedule and train volunteers to lead workshops and fill Community Center reception desk.
Provide volunteers with regular supervision, updates and feedback.

Management I & R, CyberCenter, and Library Services:
Oversee  DAP's Information and Referral Services, including InfoLink, the online community database;  coordinate regular updates to I &R information.
Coordinate upkeep of CyberCenter equipment and software.
Ensure DAP has library materials that meet community needs.
Write monthly report that includes information and referral and center usage statistics.

Coordinate Arts Programs and Social Events :
Develop and implement arts or social events to be held at the center, for diverse audiences.
Develop community collaborations to co-sponsor arts and social programming at the center.

Collaborate with Other Departments :
Work with Education & Prevention department on planning trainings and events that focus on providing HIV education and outreach to the community.
Work with HIV Health Center on incorporating our health care services information to the community.

Other Duties :
Act as part of management team, including attending lead staff meetings

Qualifications:

Minimum Experience

Five or more years of experience in Psychosocial Counseling;
Three years of progressively responsible experience in a community service or other non-profit organization, or related field;
Experience working with volunteers to implement programs.

Preferred Experience

Two years of full time experience planning events or in a related field.
Bachelors degree

Necessary Knowledge and Skills

Ability to work effectively with diverse groups of people.
Ability to prioritize assignments to complete work in a timeline manner.
Working knowledge of the principles and practices of volunteer management.
